Eagle Athletics/Eagle Club Registration Form

Show us your Eagle Spirit and become a member of the Eagle Club Athletics by registering your child in sports. Our athletic program welcomes athletes from St. Anthony Catholic School.

After completing this registration form you will receive an invoice with Eagle Club dues from FACTS.

Requirements to participate include (applies to ALL SPORTS):

Complete all required forms.
Complete the online form below one per athlete (include all sports the athlete is interested in).
Paid in full Eagle Club membership dues of $50.00 per athlete, subject to change.
